چکیده

The objective was to analyse whether the activity of the cytochrome P450 isoenzyme CYP3A4 is altered by disease activity of rheumatoid arthritis (RA). Urinary 6b-hydroxycortisol excretion, expressed as a fraction of the urinary creatinine output, was measured in 21 patients with RA treated with three different disease-modifying anti-rheumatic drugs (DMARDs) over 24 weeks. There were no correlations between urinary 6b-hydroxycortisol/creatinine (6b-OHC/Creat) ratio and measurements of disease activity such as plasma viscosity, Ritchie articular index and early morning stiffness. In addition, the three DMARDs sulphasalazine, sodium aurothiomalate and -penicillamine, smoking and the intake of various CYP3A4 substrates had no consistent detectable effect on the 6b-OHC/Creat ratio. There is no evidence that the dosage of drugs metabolized by the CYP3A4 isoenzyme needs to be adjusted for disease activity in RA.

Urinary Excretion of Acid Mucopolysaccharides by Patients with Rheumatoid Arthritis'

Several methods have been described for the determination of urinary acid mucopolysaccharides (1-4). It has been found that in normal individuals the daily urinary excretion varies with age and sex (4, 5). A few attempts have been made to correlate urinary excretion of acid mucopolysaccharides with different disease states. Craddock and Kerby (6) have reported that patients with impairment of r...

Increased incidence of urinary tract infection in patients with rheumatoid arthritis and secondary Sjögren's syndrome.

The incidence of lower urinary tract infection in 120 women with rheumatoid arthritis and secondary Sjögren's syndrome was evaluated retrospectively. Thirty one patients (26%) had secondary Sjögren's syndrome. Recurrent urinary tract infection was significantly more common in these patients (11/31) than in patients without Sjögren's syndrome (4/89). Habitual leucocyturia was also more common in...

Amino acid studies and clinical findings in normal adults and rheumatoid arthritis patients treated with ACTH.

Previous publications .from this laboratory ( 1, 2) have reported that rheumatoid arthritis patients who were treated with ACTH (adrenocorticotrophic hormone) and cortisone (17-hydroxy-11dehydrocorticosterone) exhibited changes in the urinary excretion of certain amino acids. Similar results were found to occur during remission induced by pregnancy (3, 4), and one rheumatoid arthritis patient i...

Microalbuminuria in patients with rheumatoid arthritis.

OBJECTIVES To assess (a) the prevalence of microalbuminuria in patients with rheumatoid arthritis, (b) the association between urinary albumin excretion and disease activity as estimated by the erythrocyte sedimentation rate and C reactive protein (CRP), and (c) the association between urinary albumin excretion and treatment with antirheumatic drugs. METHODS Sixty five patients with rheumatoi...
